What is a Tongue-Tie or Restricted Lingual Frenulum (RLF)?

Tongue-tie, also known as ankyloglossia is a condition in which a person’s tongue remains attached to the floor of their mouth. This occurs when the lingual frenulum (a thin strip of tissue connecting your tongue and the floor of your mouth) is shorter than usual. A short frenulum can restrict your tongue’s movement.

Ankyloglossia is most common in newborns and young children. It is commonly associated with breastfeeding  difficulties and it is also believed to have some impact on speech and/or oral hygiene problems. The true impact of tongue-tie on speech isn't clearly understood. The child's tongue needs contact with the roof of their mouth when pronouncing the lingual sounds “t”, “d”, “z”, “s,” “th,” “n,” and “l.” When their tongue is severely restricted and can't reach the roof of their mouth, the child can have problems with articulation (pronunciation).

Tongue-tie is a congenital condition, which means people are born with it.

What’s the difference between anterior tongue-tie and posterior tongue-tie?

An anterior tongue-tie is in the front of the mouth near the tip of the tongue, just behind the gums. It often looks like a thin web.

A posterior tongue-tie (also known as submucosal) is further back where the floor of the mouth meets the tongue, making it more difficult to see. This type of tie  tends to present slightly later than anterior ties.

Anterior tongue-ties are more common than posterior tongue-ties.

Who does tongue-tie affect and how common is it?

Anyone can develop tongue-tie. In some cases, ankyloglossia is hereditary (meaning it runs in families). Tongue-tie mostly affects newborns and young children. But it’s possible for older children and adults to have the condition if it has gone undiagnosed or untreated when they were babies.

Ankyloglossia occurs in up to 10% of newborns (depending on the study and definition of tongue-tie).

Does tongue-tie resolve itself without treatment?

In some instances, children learn to adjust to tongue-tie as they grow older. But if ankyloglossia is causing problems, it’s best to treat it early, as some symptoms worsen with age.

What are the symptoms of tongue-tie?

Symptoms of tongue-tie range from insignificant to significant meaning it either affects feeding or it doesn’t . Your child's tongue may appear to be heart-shaped or may have a notch in it. It may also appear to be spade like and sit flat in the bottom of the mouth.

Newborn tongue-tie may result in:

  • Difficulty latching when breastfeeding.
  • Breastfeeding for extended periods of time.
  • Falling asleep on the breast after only a few sucks/minutes
  • Constant hunger.
  • Trouble gaining weight.
  • A clicking sound during feeding.
  • If the baby is feeding from a bottle they may be dribbling and losing lots of the milk from the corners of their mouth.

If  breastfeeding, in order to create an adequate seal, the baby must extend their tongue over their jaw line. If the baby has tongue-tie, this isn't possible. They often attempt to use their gums to keep the nipple in their mouth during breastfeeding. This can be painful.

A woman may also have other symptoms related to the baby’s tongue-tie, including:

  • Cracked, sore nipples.
  • Pain during feeding.
  • Insufficient milk supply due to poor milk transfer.

How can I make an appointment?

At Private Midwives we have a range of specialist practitioners who can provide this service at home or in a clinic setting depending on their own practice preferences. Typically, this service can be arranged at very short notice due to having practitioners all over the UK and ROI.

How is tongue-tie assessed and treated?

Assessment: During the appointment the practitioner will take a history and then they will look inside the baby’s mouth and physically assess the mobility and function of the tongue. They will be looking at the extension, lateralisation, and elevation of the tongue specifically. Once assessed most of our practitioners will offer division there and then as part of the same single appointment. These appointments typically take between 1 and two hours.

Treatment: The Private Midwives’ specialist practitioner will perform a small surgical procedure where they cut the lingual frenulum with a pair of scissors specifically designed for this purpose. This is called a frenotomy (also known as frenulotomy, or tongue-tie division). This service can be performed in either a clinic setting or in your own home.

Tongue-tie surgery causes minimal discomfort for infants and is done without the need for anaesthesia. Some parents may choose to give paracetamol prior to the procedure if the baby is old enough for over-the-counter medication.

As with any surgical procedure, a tongue-tie procedure carries risks of complications, including:

  • Bleeding. (1in 3-5000)
  • Infection. (1in 10,000)
  • Injury to the saliva ducts in your mouth. (Very rare)

What about aftercare?

After the procedure is performed our practitioner will demonstrate and talk through some exercises that will help to optimise the chances of a successful division which hopefully leads to a better feeding journey. They will also assist with feeding positions and latch. You will then have access to the practitioner for at least a week after the division via WhatsApp .

How much does it cost?

Our Tongue Tie service is split in to 2 parts:

  1. Assessment
  2. Division if needed

Assessment costs:

The assessment appointment costs £150 (€150 in Ireland) and is payable in advance.

  • Review of anatomy of the baby's mouth
  • Full physical assessment of baby's  tongue function and mobility
  • Review of feeding patterns, problems, weight, centile issues
  • Attachment and positioning assessment to include feeding support

Division costs:

After the assessment, the practitioner will advise you on the best treatment. This may include division of the tongue tie. This is a small surgical procedure. Often, this can be done at the same appointment. Occasionally, a separate appointment is needed. There is an additional cost of £100-£200 (€150 in Ireland). The cost can vary depending on the type of procedure needed, the location of the appointment and the after care required. This is payable on the day.

  • Division of the tongue tie
  • Advice on aftercare, tongue exercises
  • Development of a feeding plan if needed
  • A copy of the clinical information for you to take away

Availability & Location

We can usually cover most areas. We have minimal coverage ion Scotland, Wales and Northern Ireland. If we do not have a practitioner in your area, we may still be able to offer you a home visit with a practitioner travelling to you but the cost may be higher. We will explore this for you at your request.

Your appointment may be in a clinical setting or we may be able to offer a home visit. This will vary depending on what the practitioners in your area offer. Appointments are normally within a week.

Please note, we do not divide:

  • lip ties
  • babies over 12 months of age
  • babies who have a bleeding disorder or there is a family history of bleeding disorder
  • babies with a known heart condition
  • babies having on-going medical treatment

In accepting the appointment, you confirm that none of the above applies to you. If you attend the appointment and one of these criteria is present, your appointment cannot go ahead and you will not be refunded.

I took the plunge to become a private midwife in 2021 and prior to this I had worked within the NHS for most of my adult life. Making the decision to become an independent & private midwife was not an easy decision for me to make. However, I had concluded that trying to provide care within the NHS, in a way that was individualised and that I felt allowed my families the time to feel well supported, was becoming increasingly difficult.

I can safely say, that since becoming a private midwife in 2021, I have not looked back!

As a private midwife, I have had the pleasure of caring for many women and babies throughout their pregnancies and postnatal period and I have been fortunate enough to look after some of my original clients, during their subsequent pregnancies.

I am a Mum myself and have three adult children and now, two grandchildren. I enjoy travelling, weekend trips in our VW camper and long walks with my husband and our wirehaired Dachshund, Lulu.

All midwifery and tongue tie appointments are carried out in the comfort of your own home. Appointments are never rushed. I feel that having the space to discuss any worries or concerns that you may have, can have such a positive impact on your pregnancy, birth and that precious time with your baby.

From the start of my midwifery career I was able to lead innovation, fundraising for a pool and supporting waterbirth training in hospital and community. I was part of the BUMPS a NHS group practice scheme offering continuity of care working across hospital boundaries. I later took on a consultant role providing specialized care for homeless, abused and asylum seeking women negotiating multi agency support for them, particularly a deep understanding of safeguarding processes. It also gave me a knowledge of FGM and the challenges this presents. I worked with a Obstetric consultant and liaised with other specialists to offer a support clinic within the hospital.

Breastfeeding was always a passion and I later became the infant feeding lead achieving baby friendly status and providing training and audit to all the hospital staff. I qualified as a Newborn examiner and became interested in tongue tie and how this would inhibit a babies ability to breastfeed, why women gave up and how breastfeeding could become more predominant. Working with Private midwives has enabled me to support many women and utilize all of these skills but still have time to pursue my hobbies which are dog showing and agility, pottery, swimming, crafts and gardening. I am able to spend time with my grandchildren teaching them life skills like cooking and sewing and creative skills like pottery and sculpture.

She is able to refer you to other health care practitioners who can offer body work on your baby to release any tension which may be presenting in the soft tissue, and local feeding support groups.

My name is Marguerite Williams, and I am a registered and practising midwife, International Board-Certified Lactation Consultant and Tongue Tie Practitioner.

I am compassionate and committed, with over a decade of experience providing the full range of maternity care in the UK. I’ve previously worked in the NHS primarily in the community offering antenatal care, antenatal education, homebirth care, hospital birth care and postnatal care at home.

I have a keen interest in supporting all things infant feeding as a lactation consultant. I have expertise in assisting women to reach their goals in breastfeeding, including natural term breastfeeding, physiologically led bottle feeding and gentle weaning.

I am fully insured and I operate under the credentials given to me by the International Board of Lactation Consultants, under the guidance of the Association of Tongue Tie Practitioners and am regulated by the Nursing and Midwifery Council UK.

Hi, I’m Polly and I am passionate about helping families with all feeding issues!

I have worked for many years as a birth centre and labour ward midwife and my specialism is in infant feeding. I am a private antenatal teacher and have trained as a lactation consultant and tongue tie practitioner I am also an International Board Certified Lactation Consultant (IBCLC). I have also breastfed two babies (one with a tongue tie) and know firsthand the challenges that come with it. I think all mothers/fathers/parents are incredible!

I can see you in the comfort of your own home for one-to-one breastfeeding support, tongue tie assessment and division if required. I cover most areas in Surrey, Hampshire, West Sussex and some areas of West/South London.

Hi my name is Rebecca, I have been a midwife since 2004 with a chosen specialism in breastfeeding and postnatal care. I am a trained tongue tie practitioner and registered with the Association of Tongue Tie practitioners.
I have four beautiful children that I have breastfed and I know only too well the challenges of breastfeeding, having multiple young children and babies with tongue ties! It is hard work but it is achievable and absolutely worth it, and I am here to help you every step of the way. I am passionate about seeing women and their families achieve a successful breastfeeding journey, learning together and growing a strong family bond. I will provide you with one to one support in your own home. I work in North East Hampshire, Berkshire and Surrey.

I am based in Suffolk, offering in home postnatal care and lactation services including tongue tie release in Suffolk, Cambridge, Essex and Norfolk.

I am a mother of 4 living in Bury St Edmunds. I have 5 dogs and love walks in the countryside with them.

I have spent my whole career working with Mothers and babies. My move into an NHS role began in 2011 working in a Neonatal Intensive Care Unit (NICU) with premature and unwell babies. A large part of my role was around breastfeeding and assisting new babies to learn to breastfeed under challenging circumstances.

Following a move to Midwifery I began my 11 year career at The Rosie Hospital at Addenbrookes. For the last 7 years I have been based in postnatal care which I consider my passion and I’m currently involved in management of a busy postnatal ward. My love for offering breastfeeding support continued and I am now an International Board Certified Lactation Consultant and more recently expanded this to add Tongue Tie assessment and release in the private sector.

I feel being able to support new families after birth to be a privilege and would love to hear from you if you are requiring Tongue Tie release, feeding support or even just a more personalised service in the early postnatal period.

After completing her family, Sharon commenced a Midwifery Degree at The University of Southampton. Graduating in 2005, she embarked on a career in the private sector, offering continuity of care as an Independent Midwife. In 2017 Sharon joined Private Midwives, where she has held the roles of Line Manager and Practice Development Co-Ordinator and is currently a Matron.

Sharon is an experienced Tongue Tie Practitioner and has facilitated in house training, passing her knowledge and skills on to new practitioners. She is also a NIPE practitioner and is currently undertaking further education in Mental Health Awareness.

Sharon’s passion for women-centred care has never wavered. She is proud to continue offering gold standard care as a valued member of the Private Midwives family.

Hello, I’m Suzanne and I’m a midwife, lactation consultant (IBCLC) and tongue tie practitioner. I qualified as a midwife in 2009 and worked in central London hospitals for the first part of my career but after having difficulties feeding my first child, who had a tongue tie, it ignited my passion for infant feeding and my career has been focused in this area ever since.

I am based in Rickmansworth but I offer tongue tie assessment and division in the comfort of your own home and I am happy to travel for home visits in Hertfordshire, Bedfordshire, Buckinghamshire and parts of North West London.
